SAR or subject access
Requests for medical records must be put into writing and you can do this via completing a Subject Access Request (SAR) form. These requests can take up to one month to be processed and we will need to see valid photo ID before giving you the record.
For adults, only the person whose medical record the request for information is regarding will be provided with this, unless we have written and signed consent from that person who has also shown ID to say otherwise.
For children under the age of 11, the legal parent or guardian can request access to the child's record via the same form. We will need to see photo ID of the parent or guardian only, unless we have written and signed consent from that person who has also shown ID to say otherwise.
For children over the age of 11, the legal parent or guardian can request access to the child's record via the same form but the child will need to sign this form to consent for the record to be shared.

Fees
Some work is considered to be non-NHS work meaning it is non compulsory for the Practice to complete and can come with a charge.
If you are a patient of the Practice and wish for some Non-NHS work to be completed then please do let us know. Put details of your request in writing, preferably via AccuRx.
We will then confirm if this work can be done and if it will come with any charge.
