Registration:
All new patients wishing to register must complete a registration form and fill out a Health Questionnaire. To save time when registering, you can complete the forms prior to your visit to the practice.
Please note all registrations must be made at the practice in person, a completed registration form and health questionnaire as well as the documents detailed below are required for registation. To avoid congestion at busy times registrations can only be made at the following times: -
King Harold's Way Surgery - Monday, Tuesday and Wednesday between 2:30 pm and 5:00pm
Erith Health Centre - Monday, Tuesday, Wednesday and Thursday between 3:00 pm and 5:00pm
Before registration is processed, it is the Bexley Medical Group’s policy that all new patients attend a health screening carried out by our Health Care Assistant; the screening will take approximately 10 minutes.
No registrations will be completed unless you have attended the Health Screening.
You must bring your completed health questionnaire and a urine sample with you to your appointment; the receptionist will provide you with a sample bottle when you make your appointment.
All new patients wishing to register need to bring photo ID, which will be photocopied, this could be one of the following:
New patients also need two proofs of address:
Council tax statement
Utility bill
Bank Statement
Please note: For all children under the age of 16 years you will need to provide proof of identity and where appropriate the child immunisation red book.
Valid proof of identity:
- Passport
- Birth Certificate
